The Mysterious Episodes of Mary: A Case Study on Neuroanatomy

Episode 1

Mary Lazarro, a 44-year-old mother of two, made an appointment with her physician after experiencing a prolonged episode of numbness in her chin and lower lip. Two days prior to her appointment, she felt a prickling sensation like pins and needles at the right corner of her mouth. The sensation extended to her lower lip and chin. The examination revealed only a superficial hypoesthesia of the chin and lower lip (numb chin syndrome). There was no clinical evidence of palpable regional lymph nodes or other systemic or neurologic abnormalities. Her physician scheduled her for a CT scan of the affected region. These tests showed no abnormalities in the jaw, neck, or pharynx. The numbness and hypoesthesia spontaneously disappeared gradually over a few weeks.

1. Abnormalities In the 1980s, it was generally believed that congenital abnormalities affected about 5% of the nation’s children. Some people believe that the increase in the number of chemicals in the environment has led to an increase in the incidence of abnormalities. A recent study examined 384 children and found that 46 of them showed signs of an abnormality. Is this strong evidence that the risk has increased?

2. Educated mothers The National Center for Education Statistics monitors many aspects of elementary and secondary education nationwide. Their 1996 numbers are often used as a baseline to assess changes. In 1996, 31% of students reported that their mothers had graduated from college. In 2000, responses from 8368 students found that this figure had grown to 32%. Is this evidence of a change in education level among mothers?

Is “not wanting to rock the boat” a sufficient excuse for the administration to do nothing further?

Is “not wanting to rock the boat” a sufficient excuse for the administration to do nothing further?

Health Policy

In each case, answer the questions at the end of the case and give researched references to support your assertions; also, explain what would be the ethical course of action and the legal requirements for action in the case.
Case One
Mary Stokes is in need of a kidney transplant, and her parents and siblings have been tested for compatibility. Her father is afraid of operations and knows that kidney trouble runs in the family. Before the test, Mary’s father tells the doctor that he does not want anyone, especially his wife, to know that he is compatible. He explains that if the family knows they will pressure him into being a donor. The father turns out to be the only one who is compatible. Mary asks the doctor, “Are you sure no one in my family is compatible?”
Is the father a patient and protected by confidentiality? Even if he is not a patient, is his explicit request, which was not refused, a protection of his confidentiality? If the matter is confidential, what can the physician say or do to protect the secret?
Case Two
Dr. Curious has a habit of wandering around the hospital and looking at the records of friends who are in the hospital. The nurses have tried to stop him, but he has retaliated by making their lives miserable and belittling them in public at every opportunity. The nursing administration has been notified but has done nothing, as it wants to avoid rocking the boat.
What are the nurses’ ethical obligations after they have done everything mentioned in the text? See Chapter 2 (Garrett). Is “not wanting to rock the boat” a sufficient excuse for the administration to do nothing further?

I developed severe and apparently permanent adverse reactions as a result of the Hepatitis B vaccine.

About 4 years ago I was completely healthy and very athletic before this vaccination and have now

suffered severe, debilitating autoimmune side effects from this vaccine. I get joint pain when I

lift weights in the gym, only on one side of the body, not in both arms at once, just the left.

I never used to get joint pain when I was lifting weights before the vaccine,

I used to lift 100KG but now I get joint pain when I lift 50KG.

I also have joint pain in two of my middle toes on my right foot especially when I walk on the stairs

bare feet. There’s a measurable loss in stamina before the vaccine to how I am now, I experience breathlessness

quicker when I do cardiovascular exercise then before I had the vaccine.

I have chronic fatigue and it is not improved by getting rest.

I have a dry persistent cough when there’s nothing to bring up. I have abdominal pain on the left side

of the stomach sometimes when I sit down or lie down upright. I experienced Psychosis due to neurologic conditions.

I used to think the TV was communicating with me.

I suffered the residual effects of these symptoms for more than 4 years.

Lupus-like syndrome, multiple sclerosis-like syndrome, rheumatoid arthritis.

Tests:

Acute Disseminated Encephalomyelitis (ADEM)

Transverse Myelitis

Guillain-Barré Syndrome (GBS)

Lupus

Brachial Plexitis

Myopathy

Quadraparesis

Postural Orthostatic Tachycardia Syndrome (POTS)

Chronic Inflammatory Demyelinating Polyneuropathy (CIDP)
